DE-IDENTIFIED RECORD — NO PATIENT IDENTIFIERS BELONG HERE.

De-identified by construction

Administer, score and chart the PID-5 on your own bench.

Trait Bench gives a clinician the whole loop for the Personality Inventory for DSM-5: hand out the form, key in or paste back the responses, and read a scored profile across 25 facets, 5 domains and 7 AMPD trait composites. No account, no subscription, and nothing you enter is stored on a server.

Your cases live in your own browser and nowhere else. The server renders each page from what your browser sends and keeps no copy; the privacy page says exactly what that means.

What it does

01

Administer

Both published forms: the 220-item full version and the 100-item short form. On screen item by item, or as a dense grid for keying in a paper form somebody already filled out. Items appear verbatim in published order, never shuffled, reworded or branched.

02

Score

25 facets, 5 domains and 7 AMPD trait composites, computed from the published key with reverse-scored items handled. Missing responses are carried as missing rather than counted as zero, and a facet with too few answers is suppressed instead of guessed.

03

Chart and print

A banded profile chart and a domain bar chart, plus three printable documents: a blank questionnaire, the completed record showing every response given, and the scored report. The blank questionnaire also downloads as a typeset PDF, ready to hand out.

Paper in, profile out

Most of this instrument's life is on paper. Print the blank form, hand it over, and get the responses back in without keying 220 cells by hand: photograph the completed sheet, have your own assistant transcribe it, and paste the result straight in.

Every paste is previewed before anything is stored. The tool shows what it read, what it could not read, and which items are still missing, and it refuses a bare list of values whose length does not match the form, because one dropped value would shift every response after it.

What it deliberately will not do

A measure that overstates itself is worse than no measure. These are refusals built into the tool, not settings.

No T scores, no percentiles

The PID-5 metric is a bounded 0 to 3 item mean with several facets resting on their floor. That does not support the normality a T score implies, so none is produced. Facets are shown against the published 1.0 and 1.5 SD cut-offs and labelled in bands.

No diagnosis

The instrument reports trait elevations against a reference sample. It does not emit a disorder label or a diagnostic threshold, and this tool will not display one. The composites are profile similarity scores and are shown as such.

No name fields, and no server copy

A case holds item responses, a code the tool assigns, a Case ID you choose under a fixed alphabet, and the year. There is no name field, no note field and no date field, and every date is cut back to its year before it is stored. Whatever you enter stays in your browser; the security page is exact about which part of that the software enforces and which part is yours.

Built for de-identified use No name, note or date fields. The map from a case code to a person is yours, in your own records, and never here.
